Should you experience a workplace accident, be sure to fill out the Supervisor Incident Report form and distribute the Employee Incident Report to the injured employee.

It is imperative that ANY incident or accident be reported immediately. If there are any witnesses to the incident/accident, please have them complete a statement – who, what, when, where, why.
